a) Data Collection Form：

b) Passport Sized Photo - Upload at 14

c) Baptism application form - Upload at 12

d) A Written Salvation Testimony

Your testimony can be titled “My Story of knowing Jesus my Saviour” and you can start with the following introductory statement: “I understand that

Baptism is an open declaration of my faith in Jesus. My story of knowing Jesus as my Saviour is as follows...”

Guidelines for your testimony:

• What your life was like before you knew Christ

• How you came to know Christ

• How has your life changed after following Christ

d) NRIC/FIN/Work Permit:

Upload a coloured image of the front and back of your NRIC/FIN/Work Permit form at 15.

3) Your name will appear in the Baptism Week bulletin. Please indicate your preferred name for publication (PDPA compliance). 您的名字将会刊载在我们的周报上，请注明您希望被刊载的名字。

6) Citizenship 国籍

7) Date of Birth (DD/MM/YYYY) 生日日期 (日日/月月/年年年年）

9) The church's position is baptism by immersion. However, if you have a specific reason for choosing sprinkling, please let us know. 教会鼓励浸水洗礼。不过，如果您有特别的理由必须选择洒水洗礼，请列明原因。

10) As baptism is a public declaration of your faith, you are encouraged to record your testimony via video or audio. Please let us know if you would be willing to do so. 洗礼是公开宣告你的信仰，我们鼓励你通过视频或音频记录你的见证。请让我们知道您是否愿意为得救见证进行录影/录音？
