*Criteria for membership -
1) Membership in Gospel Light Christian Church is only for those who have undergone believer's baptism and must be age 16 and above. Parental consent is required for membership applicants below 21 years of age.
2) All participants/applicants are required to view 4 videos, answer the quizzes and attend a mandatory 2-session class. Attendance at the Gospel Light Membership Class (Part 1 & 2) is a prerequisite for membership application, unless otherwise exempted by the Senior Pastor.
3) Applicants who are interested in applying for Membership have to submit the respective documents as required.
4) An informal 15-minute one-on-one meeting with the pastor will take place after points 2 and 3 have been completed.
5) After the informal meeting, Membership will be confirmed upon approval by our Board of Elders and your subsequent signing of the church register.
≥ 16 years old to < 21 years old If you are between 16 years old and below 21 years old at the point of submitting this form, we will require you to seek parental consent to allow you to attend the class.

3) I hereby declare that the details furnished above are true, accurate and complete to the best of my knowledge and belief and I undertake to inform you of any changes therein, immediately. In case any of the above information is found to be false or untrue or misleading or misrepresenting, I am aware that I may be held liable for it.
